{"id":1698,"date":"2018-06-27T13:13:31","date_gmt":"2018-06-27T12:13:31","guid":{"rendered":"http:\/\/ldworguk.creo.co.uk\/training-events\/what-matters-to-me\/"},"modified":"2019-05-08T12:38:30","modified_gmt":"2019-05-08T11:38:30","slug":"what-matters-to-me","status":"publish","type":"page","link":"https:\/\/www.ldw.org.uk\/cy\/training-events\/what-matters-to-me\/","title":{"rendered":"Beth sy\u2019n bwysig imi"},"content":{"rendered":"<p>O dan y Ddeddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014 mae yna reolau newydd ynghylch sut y dylai awdurdodau lleol asesu pobl ar gyfer gofal a chefnogaeth a llunio a darparu cynlluniau gofal yn seiliedig ar eu hasesiadau.<\/p>\n<p>Mae cynnwys yr unigolyn yn y broses asesu a chynllunio gofal yn hanfodol i&#8217;r rheolau yma er mwyn sicrhau bod eu nodau llesiant yn cael eu cydnabod a bod gofal a chefnogaeth sydd yn briodol i ddiwallu&#8217;r nodau yma yn cael eu sefydlu.<\/p>\n<p>Mae Anabledd Dysgu Cymru wedi derbyn Grant Trydydd Sector Gwasanaethau Cymdeithasol Cynaliadwy Llywodraeth Cymru i gynhyrchu deunyddiau Hawdd ei Ddeall a chynnal sesiynau hyfforddi\/ymwybyddiaeth. Fe fydd y deunyddiau a&#8217;r hyfforddiant yma yngl\u0177n \u00e2&#8217;r system asesu a chynllunio gofal ar gyfer pobl ag anabledd dysgu gyda&#8217;r nod o wella eu hymgysylltiad a&#8217;u cyfranogiad yn y broses.<\/p>\n<p>Fe fydd Beth sy&#8217;n Bwysig imi yn rhedeg tan Mawrth 2020 ac yn ystod y cyfnod yma fe fyddwn yn:<\/p>\n<ul>\n<li>cynhyrchu deunyddiau Hawdd ei Ddeall ar asesu a chyllunio gofal<\/li>\n<li>datblygu sesiynau hyfforddi<\/li>\n<li>darparu hyfforddiant i bobl ag anabledd dysgu<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>O dan y Ddeddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014 mae yna reolau newydd ynghylch sut y dylai awdurdodau lleol asesu pobl ar gyfer gofal a chefnogaeth a llunio a darparu cynlluniau gofal yn seiliedig ar eu hasesiadau.
