Carers Wales asked us to make an Easy Read Guide for carers about looking after someone when they come out of hospital. Carers Wales are working to make life better for carers and it is important that their information is accessible to everyone.

We also made a Welsh Easy Read version of the guide. Both guides are digitally accessible for screen readers and other assistive technology.
