The Learning Disability, Autism and Neurodevelopmental Team in Welsh Government has written this questionnaire.
They want to know more about people’s experiences with neurodevelopmental conditions.
Please answer the questions if you:
• are on the waiting lists for a diagnosis
• or have already had a diagnosis.
Welsh Government is working on writing a National Neurodevelopmental Action Plan. The information they get through this questionnaire will help them write this plan.
