Under the Social Services and Well-being (Wales) Act 2014 there are new rules about how local authorities should assess people for care and support and draw up and deliver care plans based on their assessments.
Fundamental to these new rules is that the individual should be involved in the assessment and care planning process to ensure that their well-being goals are recognised and that care and support appropriate to meeting these goals is put in place.
Learning Disability Wales has received from the Welsh Government’s Sustainable Social Services Third Sector Grant to produce Easy Read materials and run training / awareness sessions. These materials and training will be for people with a learning disability about the assessment and care planning system with an aim of improving their engagement and involvement in the process.
What Matters to Me will run until March 2020, during this time we will:
- produce Easy Read materials on assessment and care planning
- develop training sessions
- deliver training to people with a learning disability
