The Older People’s Commissioner for Wales asked us to make an Easy Read version of their new guide. It is about getting the help you need when you are leaving the hospital.
It explains what happens when you are discharged from hospital and what care and support you can get after you leave.
It provides useful information about your rights when you leave hospital, as well as details of organisations that can help.
We also made a Welsh Easy Read version of the guide. Both are digitally accessible for screen readers and other assistive technology.
